Our Intake Department is the first point of contact for services at Youthville Family Consultation Service. Intake staff will assess your needs, schedule the first appointment, instructions for online form use, verify insurance benefits and provide information regarding private pay fees. When necessary, our intake department staff can make appropriate referrals and offer resource information. For your first appointment call 264-8317 and select option #2.

The adult and child questionnaires include information that is helpful in determining how best to serve you. Please choose the appropriate form and complete all questions. If you do not know the information or the information does not apply to you, please indicate.

We accept most major insurances as well as Medicaid, Medicare and Cenpatico. As a part of the intake process, you will be asked to provide a copy of insurance coverage at your first appointment and monthly thereafter.
