Most major health insurances are accepted, including Medicaid, Medicare and Cenpatico. For those without insurance, we offer a self-pay fee.
Please take a few minutes to print and complete these forms. Bring them with you to your first visit, or come a few minutes early to complete them at the office. If you have mental health coverage for a plan that we are on, feel free to call, fax, or email us your insurance information.
This will allow us to review your benefits, or gain any necessary precertification from your insurance company prior to your first appointment.
Directions:
Take Kellogg or HWY54 to the Seneca exit. Take Seneca street going North until you reach McLean Boulevard turn left or travel West on McLean one block until you see Dodge street. Youthville Family Consultation Service's main parking lot will be on the South (left) side off of McLean Boulevard and Dodge street. Handicap parking is located in a small lot connected to the building just off of Exposition street.
